How can I change or withdraw my information from the New York State Donate Life Registry?

You can modify your gift.

Click on the following link: donatelife.ny.gov/log-in and log in; or
Complete, sign and mail the Change and Specification Form to the address on the form or email to Donate Life New York State at: registry@donatelife.ny.gov.

You can withdraw from the Registry.

Click on the following link: donatelife.ny.gov/log-in and log in; or
Complete, sign and mail the Removal Form to the address on the form or email to Donate Life New York State at: registry@donatelife.ny.gov.
